Support at Home Program Getting Started Linking you with Self-managing providers At Aged Care Ambassadors, we work alongside you to connect you with approved providers who support self-managing and offer low transparent fees. Our role is to make sure you keep choice and control without the overwhelm. We can help you: Identify reputable self managing providers that match your goals, location and budget. Compare fees, inclusions and contract terms in plain language so you know exactly what you're signing. Set up your package with your chosen provider, including budgets, service types, and reporting. Build a team of support workers (cleaning, personal care, nursing, allied health and social support) that fits your lifestyle. We provide regular check-ins to ensure the supports are meeting your requirements. as this is a noted gap from analysis conducted. We review your services regularly and help switch providers or workers if your needs change. You can call us and speak directly with us at anytime. Believe that ageing shouldn't mean giving up control. The Support at Home Program put's the power in your hands, helping you design care that suits your lifestyle, goals, and budget. You approve decisions and remain in control with us doing the heavy lifting. Competitive fees We believe high quality support should also represent good value. Care coordination and scheduling services are offered at a highly competitive rate of 2.5% of package with a cap of 1.5 hours of supports a month or $90 per hour depending on your requirements *special time limited for new clients signing up until 1st July 2026. We focus on efficiency and transparency, so you get the maximum benefit from every hour you purchase. Wherever possible, we work with you and your provider so our services can be funded from your home care package , inline with program rules. Funding The new Support at Home Program introduces eight funding classifications - with each annual budget allocated in quarterly amounts. Rules for unspent funds If you’re already receiving a Home Care Package, any unspent funds will move with you into the new Support at Home program and remain available for your care. You can continue to use your existing balance to top up approved services when your regular quarterly budget has been used, or to pay for agreed items such as assistive technology and home modifications that meet your assessed needs. Our Services Our local and friendly team can provide you with the following supports Care Management Services All registered providers are required to deliver care management—no matter if you choose full-service management or self-management of your home care supports. What is Care Management Fees Care management is limited to 10% of your Support at Home budget. This funding is pooled among participants, allowing providers the flexibility to offer more support during peak needs—like health crises—and scale back when your requirements are minimal. This approach ensures timely, tailored care whenever you need it. What is Self-Management For those seeking greater independence, the self-management option puts you in full control of your care. You select, hire, and manage your own service providers to match your care plan. We can connect you with a registered provider, plan-manager, self-managed provider or your preferred choice to ensure seamless integration. Plus, download our free checklist to simplify switching providers. Once you register your interest, we'll contact your self-managed or plan-managed provider to handle compliance checks and link in as your care partner. When you chose to self-manage your home care package, you choose to stay in charge. Services you want You are uniquely qualified to understand the services and support you require. By managing yourself, you can prioritise what matters most to you, allowing you to maintain your independence at home and within your community. When and where It's important to feel at ease with the people around you. By self-managing you can select individuals who speak your language and share similar interests. More hours of supports Providers fees can reduce your Home Care Package funding. Self-managed, helps you avoid these costs, allowing for more hours of support. Checklist for ending services with your current provider Your Choice, Your Support At A ged Care Ambassadors we can support self-managed clients right away by linking in with your chosen registered provider to act as your care partner, delivering tailored support worker services to meet your needs. If your Support at Home funding is provider-managed, you retain the right to select the supports and workers that best suit you. Aged Care Ambassadors support elderly people on the Sunshine Coast, Queensland, who are living successful, engaged, and meaningful lives in the community and at home. We offer comprehensive In-Home and Senior Care services, ensuring access to in-home support to help individuals stay connected and remain where their heart is. When does the new Support at Home program commence? On the 4th June 2025, the Australian Government announced that the Support at Home program will now commence on 1 November 2025, alongside the introduction of the new Aged Care Act. The program aims to support older people to remain healthy, active and socially connected to their community. In the meantime, existing programs Commonwealth Home Support Program (CHSP), Short-Term Restorative Care (STRC), and Home Care Packages (HCP) - will continue to provide support for older Australians who wish to keep living at home. For the latest information, please read the Government's media release here For the latest official updates visit: Support At Home Website What is Support at Home Program? The new Support at Home Program provides funding to help older Australians live independently at home for longer, simplifying access to aged care services through a single, streamlined system. From 1 November 2025, Home Care Packages and the Short-Term Restorative Care program will now fall under the Support At Home Program. At a later date, the Commonwealth Home Support Programme (CHSP) will also move under the Support at Home Program (no earlier than 1 July 2027). The new service list covers clinical care, independence support, and everyday living tasks - from nursing and personal care to transport, cleaning, and meal preparation. Read more about the Support At Home program here What are the new Support at Home Service Lists? What is the single assessment system? Support at Home brings in a new Single Assessment System to make the process easier. Once assessed, you'll get a personalised support plan to share with your chosen provider. Approved aged care support services will be based on your aged care assessment and eligibility. After you have been assessed, you will receive a notice of decision and an individual support plan to use with your provider. You can find out if you are eligible by visiting the assessment tool online at myagedcare.gov.au/assessment. The new program introduces eight funding levels, offering more tailored support to replace the current 4 Home Care Package levels. Older Australians receiving Home Care Packages will be assigned a budget that aligns with their current care package level. New applicants to the Support At Home program will be assessed and given a classification and budget suited to their specific needs. The indicative budget amounts are correct as at 3 February 2025. The final budget amounts for all participants will be confirmed before the start of the Support At Home program. Already on a Home Care Package? You'll be mapped to the equivalent new budget level. The program offers End of Life Support, providing an extra $25,000 over 16 weeks for care at home if you have three months or less to live. This works alongside specialised palliative care services, not as a replacement. The Assistive Technology and Home Modifications Scheme provides extra support (separate from your main funding) to help you source equipment or modify your home for safety and independence.
