The Australian Government is updating the aged care services:
From 1 November 2025: The New Support at Home Program will replace the Home Care Packages and Short-Term Restorative Care Programme.
From 1 July 2027 (at earliest): Commonwealth Home Support Programme transitions to the new program.

What is the Support at Home Program?
The Support at Home Program is Australia’s new aged care model designed to replace the current Home Care Packages (HCP) and Short-Term Restorative Care (STRC) programs. It offers flexible, quarterly funded care tailored to your individual needs and circumstances.

Your individual support plan is divided into three key categories, ensuring holistic care across all areas of your life:

Quarterly Budget Levels (Indicative)
Depending on your assessed level of need, you may be allocated one of eight budget levels, released every quarter:

Peace of mind
The Support at Home Program helps older Australians (over 65, or over 50 for Aboriginal or Torres Strait Islander people) stay independent by funding help with daily tasks like cleaning, gardening, personal care, and social support. Hubungi My Aged Care di 1800 200 422
Hubungi kami, untuk Penilaian komprehensif, yang dilengkapkan oleh Pasukan Penilaian Penjagaan Warga Emas / Perkhidmatan (ACAT / ACAS). Pastikan anda mempunyai kad Medicare anda. Jika anda memanggil bagi pihak seseorang, mereka perlu bersama anda untuk memberikan persetujuan lisan mereka sebelum anda boleh bercakap bagi pihak mereka.

Lengkapkan Penilaian Komprehensif.
An ACAT Assessor will visit you in your home and run through a range of questions to help them understand your current situation. It’s a great idea to be prepared for these questions – write down a list of all the things that you are struggling with and finding difficult. Don’t try and put on your “best face”, talk honestly about your needs so that you can get the help you deserve. Additionally, if you receive informal support, for example, from family members, write down what you wouldn’t be able to do if they weren’t there to help you.

Anda akan menerima surat daripada My Aged Care dengan keputusan Penilaian anda.
Selepas penilaian menyeluruh, tahap pembiayaan untuk pakej anda akan ditentukan. Pakej diperuntukkan berdasarkan sistem keutamaan negara. Anda akan dimaklumkan tentang tahap pembiayaan anda melalui surat dalam mel. Perlu diingat bahawa pembiayaan boleh mengambil masa 3-6 bulan untuk datang.
Anda juga mungkin telah diluluskan untuk Rehat Kediaman, atau Rumah Penjagaan Orang Tua – anda tidak perlu menggunakan perkhidmatan ini, tetapi perkhidmatan ini tersedia jika anda memerlukannya. Jika anda memerlukan penjagaan segera, tanya My Aged Care tentang subsidi kerajaan lain yang boleh anda akses semasa anda menunggu.

Tunggu Tugasan!
Jika anda diluluskan untuk Pakej Penjagaan Rumah, langkah terakhir ialah menunggu pembiayaan anda diberikan kepada anda. Masa menunggu ini boleh menjadi agak lama, terutamanya jika anda telah diluluskan untuk Pakej tahap 3 atau 4. Setelah ditugaskan, anda akan menerima surat daripada penjagaan Umur Saya yang menasihati anda tentang perkara ini. Di dalam surat ini terdapat kod rujukan unik anda, yang pembekal pilihan anda perlukan untuk memulakan perkhidmatan.
