When does the new Support at Home program commence? On the 4th June 2025, the Australian Government announced that the Support at Home program will now commence on 1 November 2025, alongside the introduction of the new Aged Care Act. The program aims to support older people to remain healthy, active and socially connected to their community. In the meantime, existing programs Commonwealth Home Support Program (CHSP), Short-Term Restorative Care (STRC), and Home Care Packages (HCP) - will continue to provide support for older Australians who wish to keep living at home. The new Support at Home Program provides funding to help older Australians live independently at home for longer, simplifying access to aged care services through a single, streamlined system. From 1 November 2025, Home Care Packages and the Short-Term Restorative Care program will now fall under the Support At Home Program. At a later date, the Commonwealth Home Support Programme (CHSP) will also move under the Support at Home Program (no earlier than 1 July 2027). Support at Home brings in a new Single Assessment System to make the process easier. Once assessed, you'll get a personalised support plan to share with your chosen provider. After you have been assessed, you will receive a notice of decision and an individual support plan to use with your provider. The new program introduces eight funding levels, offering more tailored support to replace the current 4 Home Care Package levels. Older Australians receiving Home Care Packages will be assigned a budget that aligns with their current care package level. Support at Home Program Getting Started Linking you with Self-managing providers At Aged Care Ambassadors, we work alongside you to connect you with approved providers who support self-managing and offer low transparent fees. Our role is to make sure you keep choice and control without the overwhelm. We can help you: Identify reputable self managing providers that match your goals, location and budget. Compare fees, inclusions and contract terms in plain language so you know exactly what you're signing. Set up your package with your chosen provider, including budgets, service types, and reporting. Build a team of support workers (cleaning, personal care, nursing, allied health and social support) that fits your lifestyle. We provide regular check-ins to ensure the supports are meeting your requirements. as this is a noted gap from analysis conducted. We review your services regularly and help switch providers or workers if your needs change. You can call us and speak directly with us at anytime. Believe that ageing shouldn't mean giving up control. The Support at Home Program put's the power in your hands, helping you design care that suits your lifestyle, goals, and budget. What is Care Management Fees Care management is limited to 10% of your Support at Home budget. This funding is pooled among participants, allowing providers the flexibility to offer more support during peak needs—like health crises—and scale back when your requirements are minimal. This approach ensures timely, tailored care whenever you need it. What is Self-Management For those seeking greater independence, the self-management option puts you in full control of your care. You select, hire, and manage your own service providers to match your care plan. We can connect you with a registered provider, plan-manager, self-managed provider or your preferred choice to ensure seamless integration. Plus, download our free checklist to simplify switching providers. Once you register your interest, we'll contact your self-managed or plan-managed provider to handle compliance checks and link in as your care partner. When you chose to self-manage your home care package, you choose to stay in charge. Services you want You are uniquely qualified to understand the services and support you require. By managing yourself, you can prioritise what matters most to you, allowing you to maintain your independence at home and within your community.
